Thyroid nodules are not necessarily due to iodine overload. Patients with thyroid nodules are typically classified as having either solitary or multiple nodules; some even present with bilateral thyroid nodules. The etiology of thyroid nodules is multifactorial and may involve dietary factors or external environmental stimuli. In certain cases, iodine excess may contribute to nodule formation. Therefore, for patients diagnosed with thyroid nodules, the first step is to determine the nature of the nodule. Accurately characterizing the nodule helps guide subsequent treatment decisions.

Thyroid nodules are broadly categorized as benign or malignant. Most benign nodules require no specific treatment and can be managed with regular follow-up monitoring. Malignant nodules, however, necessitate surgical intervention. Many patients with thyroid nodules remain asymptomatic and are often diagnosed incidentally during routine physical examinations. Such cases may be managed conservatively with periodic surveillance. However, if a nodule grows large enough to cause compressive symptoms—such as hoarseness, a sensation of pressure in the neck, dyspnea, or dysphagia—surgical treatment becomes necessary. Additionally, if thyroid dysfunction is present, management may involve antithyroid medications or surgery, depending on the specific clinical scenario. When malignancy is suspected, ultrasound imaging and/or fine-needle aspiration cytology (FNAC) should be performed to confirm whether the nodule is benign or malignant. If confirmed as malignant, surgical excision is indicated.
