A child born to a B-type and an O-type parent will have either O-type or B-type blood—not A-type or AB-type blood. B-type blood is more likely, with a probability of 75% (3/4), whereas O-type blood occurs with a probability of 25% (1/4). The specific genetic reasoning is as follows:

1. If the mother is O-type and the father is B-type, hemolytic disease of the newborn (HDN) may occur. HDN arises when maternal antibodies—produced against fetal blood group antigens incompatible with the mother’s own blood type—cross the placenta and trigger immune-mediated hemolysis in the fetus. While still in utero, bilirubin generated by hemolysis is largely metabolized by the mother’s liver. After birth, however, the newborn’s immature liver cannot adequately process the large amounts of bilirubin resulting from hemolysis, leading to jaundice—the hallmark clinical manifestation of HDN.

2. Regardless of whether the mother or father is O-type or B-type, their child can only be O-type or B-type—no other blood types are genetically possible. Furthermore, if the mother is B-type and the father is O-type, the likelihood of the child being B-type is higher (75%), while the chance of O-type is 25%.
