The disease has a rapid onset: initially, patients exhibit marked conjunctival edema, conjunctival hyperemia (redness), and noticeable swelling of the eyelids, often accompanied by mild edema and pain. As the condition progresses, pronounced symptoms of redness, swelling, heat, and pain emerge—eyes appear intensely red and produce abundant mucoid discharge; however, the cornea is typically unaffected. In cases involving highly virulent bacterial strains, complications may include conjunctival pseudomembranes, regional lymphadenopathy, entropion (inward turning of the eyelid), trichiasis (misdirected eyelashes), or even ophthalmoplegia (eye muscle paralysis). Notably, in patients with alcohol dependence, periorbital edema tends to be especially prominent.

Conjunctivitis is also referred to as acute or subacute bacterial conjunctivitis. Common clinical features include conjunctival hyperemia and edema, relatively infrequent corneal involvement, increased mucopurulent discharge, photophobia, epiphora (excessive tearing), blurred vision, ocular pain, and occasionally upper respiratory tract symptoms.
