Classification of Premature Cardiac Contractions
Premature cardiac contractions are classified into three types based on the location of the ectopic pacemaker site. The first type is atrial premature contraction (APC), originating from any site within the atria outside the sinoatrial (SA) node. The second type is atrioventricular (AV) junctional premature contraction, arising from the AV junction and conducting retrogradely to produce a P-wave that is inverted relative to the early QRS complex. The third type is ventricular premature contraction (VPC), one of the most common arrhythmias, originating below the His bundle branches and resulting in premature ventricular depolarization. Some premature contractions may be attributable to underlying pathological conditions, necessitating treatment of the primary disease.

During premature cardiac contraction, patients often experience characteristic symptoms, including palpitations and chest tightness, as well as sensations of cardiac “pausing” or “jumping.” On auscultation, the heart rate appears irregular, with distinct premature beats followed by compensatory pauses. Patients may also detect an irregular pulse upon self-palpation. Additional symptoms may include chest tightness, shortness of breath, and fatigue; in severe cases, patients may develop dizziness, headache, and other clinical manifestations.

For patients with more severe disease, lifestyle modifications—including dietary adjustments—may prove insufficient for effective control; pharmacological intervention is then required to alleviate symptoms. Anti-arrhythmic medications used to treat premature cardiac contractions carry significant systemic toxicity and adverse effects; therefore, patients should strictly adhere to their physician’s instructions when taking these drugs.
