Among organic cardiac conditions, coronary artery disease—specifically, coronary atherosclerotic heart disease—as well as unstable angina and acute myocardial infarction, frequently trigger premature beats due to myocardial ischemia-induced abnormalities in the cardiac conduction system. Similarly, other cardiac disorders such as myocarditis and cardiomyopathy can also cause premature beats.

Frequent late-night sleeping in daily life may lead to relatively frequent premature cardiac contractions (premature beats). What are the common causes of frequent premature beats?

What Causes Frequent Premature Cardiac Contractions?

There are two primary categories of causes: organic heart disease and non-organic (functional) heart disease. Among organic cardiac conditions, coronary artery disease—specifically, coronary atherosclerotic heart disease—as well as unstable angina and acute myocardial infarction, commonly trigger premature beats due to myocardial ischemia-induced abnormalities in the cardiac conduction system. Similarly, other cardiac disorders—including myocarditis, cardiomyopathy, and valvular heart disease—can impair myocardial function and predispose patients to premature beats. The second category comprises non-organic cardiac conditions. Many young individuals today experience chronic sleep deprivation, excessive fatigue, psychological stress, late-night habits, insufficient sleep, smoking, and alcohol consumption. These factors elevate sympathetic nervous system activity, thereby increasing the likelihood of premature beats.

When premature cardiac contractions occur frequently, initial evaluation should include standard electrocardiography (ECG) and ambulatory (Holter) ECG monitoring. For patients with underlying organic heart disease, treatment must target the primary condition—whether atrial, junctional, or ventricular premature beats—and may involve antiarrhythmic medications such as metoprolol or bisoprolol. As the underlying disease improves, premature beats typically decrease in frequency.
