What Are the Clinical Manifestations of Coronary Heart Disease?
Coronary heart disease typically refers to coronary atherosclerotic heart disease, also known as ischemic heart disease. It results from the formation of atherosclerotic plaques on the walls of the coronary arteries, causing narrowing of the arterial lumen or local thrombus formation that obstructs blood flow—thereby inducing myocardial ischemia and hypoxia. Major risk factors include hypertension, hyperlipidemia, diabetes mellitus, smoking, obesity, age, sex, and a family history of premature CHD. Due to variations in the location, extent, and severity of coronary artery lesions, CHD presents with diverse clinical manifestations.

The primary symptoms of CHD are chest tightness and chest pain. Classic anginal pain is located behind the sternum and may radiate to the left chest, left shoulder and back, and the anterior-medial aspect of the left upper arm. It may also radiate to the neck, pharynx, jaw, and head. The typical character of the pain is crushing or pressure-like, often accompanied by marked chest tightness. Some patients experience a sensation of impending doom, frequently compelling them to stop all activity immediately. Common precipitating factors include climbing stairs, brisk walking, physical fatigue, overeating, exposure to cold, and emotional stress; symptoms usually subside with rest. Some patients present with atypical symptoms, such as upper abdominal pain or discomfort beneath the xiphoid process.
