Introduction
Tinnitus and dizziness may result from cerebral ischemia occurring when the patient wakes up during the night—particularly ischemia of the internal auditory artery, which can cause dizziness, tinnitus, and possibly nausea. Patients with a history of cerebral arteriosclerosis or cerebrovascular stenosis require prompt attention and should seek medical evaluation and treatment at a hospital. Ménière’s disease may also cause tinnitus and dizziness; in such cases, patients typically experience severe vertigo.

Many people experience tinnitus—perceiving sounds such as buzzing, chirping (like cicadas), or rushing water in their ears without any external auditory stimulus. In fact, these sounds do not physically exist. This is a classic manifestation of tinnitus. Transient tinnitus generally has minimal impact on overall health; however, recurrent or persistent tinnitus warrants heightened vigilance. So, what causes tinnitus upon waking up in the middle of the night?

Causes of Tinnitus Upon Waking at Night

Tinnitus and dizziness upon waking at night may result from cerebral vascular ischemia—particularly ischemia of the internal auditory artery—which can lead to dizziness and tinnitus, sometimes accompanied by nausea. Patients with a history of cerebral arteriosclerosis or cerebrovascular stenosis must seek prompt medical attention and undergo evaluation and treatment at a hospital. Ménière’s disease is another potential cause of tinnitus and vertigo. In such cases, patients typically experience severe vertigo, possibly accompanied by visual spinning (rotational vertigo), cold sweats, and palpitations—symptoms attributable to endolymphatic hydrops (fluid accumulation) in the inner ear labyrinth. These patients require symptomatic anti-vertigo and anti-emetic therapy at the hospital, along with corticosteroid administration to reduce inner ear edema.

If tinnitus occurs during nighttime sleep, patients are advised to wear headphones and listen to soft music, which can effectively mask the tinnitus perception. Simultaneously, patients should consciously relax. Tinnitus is an extremely common clinical condition; if of recent onset, it may be reversible with timely intervention. It is advisable to initiate treatment promptly using medications that improve microcirculation and nourish neural tissue. Before bedtime, gently massaging the auricle repeatedly may help enhance local blood circulation. If tinnitus persists despite targeted pharmacological therapy, acupuncture and hyperbaric oxygen therapy may also offer therapeutic benefits.

If symptoms persist or worsen, patients should consult an otolaryngologist (ENT specialist) at a hospital for comprehensive evaluation and appropriate diagnostic testing. We hope this information proves helpful to you.
