The severity ultimately depends on the nodule’s pathological nature. If a patient is diagnosed with a BI-RADS Category 4 nodule, it is strongly recommended to undergo either a core needle biopsy or surgical excision, followed by histopathological examination to determine whether the nodule is benign or malignant—and thereby guide subsequent management.Malignant breast nodules are typically more severe, requiring not only radical surgery and lymph node dissection but also adjuvant therapies such as postoperative chemotherapy.

Under normal circumstances, the most direct contributing factor to a BI-RADS Category 4 breast nodule is endocrine imbalance. Such imbalances may manifest in women as menstrual irregularities, loss of appetite, or persistent fatigue and excessive sleepiness. From the perspective of Traditional Chinese Medicine (TCM), the primary underlying cause is stagnation of Liver Qi. Although no overt pathological changes may be apparent in the short term, this condition does increase the risk of malignant transformation.
