Introduction
Frequent ear picking can easily damage the skin of the ear canal, leading to secondary fungal infection, which may cause itching in both ears. Poor ear canal hygiene allows bacteria to proliferate within the canal or results in chronic bacterial invasion of the external auditory canal mucosa, triggering inflammatory responses in the mucosa and submucosal tissues—also manifesting as the aforementioned symptoms. Additionally, itching of the ears may occur following exposure to external allergens.

Intense itching in both ears may be caused by fungal infection, otitis externa eczema, chronic otitis externa, or allergic reactions.

1. Fungal Infection:

Frequent ear wiping or improper ear cleaning (e.g., using cotton swabs) can damage the skin of the ear canal, predisposing it to secondary fungal infection. This may result in bilateral ear itching and pain, often accompanied by purulent discharge.

2. Otitis Externa Eczema:

Otitis externa eczema may develop due to inadequate ear canal hygiene, allowing bacterial overgrowth within the ear canal. Typical symptoms include erythema and pruritus of the ear.

3. Chronic Otitis Externa:

Prolonged bacterial invasion of the external auditory canal mucosa can trigger inflammatory responses in both the mucosa and submucosal tissues, leading to bilateral ear itching.

4. Allergic Reaction:

Exposure to external allergens may provoke ear itching, often accompanied by other allergic symptoms such as sneezing, rhinorrhea, and ocular pruritus.
