Smegma contains numerous bacteria—and even fungi. If improperly managed, it may lead to balanoposthitis (inflammation of the glans penis and foreskin), hyperemia and thickening of the inner preputial surface, as well as erythema, swelling, and pain on the glans. These conditions can further exacerbate inflammation and increase irritating secretions. In severe cases, chronic irritation may contribute to penile cancer. Moreover, in men of reproductive age, pronounced smegma accumulation may cause vaginal infections in their female partners.

Smegma is a chemical carcinogen secreted by the sebaceous glands beneath the male foreskin. So, what effects does smegma have?

These conditions can further exacerbate inflammation and increase irritating secretions. In severe cases, chronic irritation from smegma may contribute to penile cancer. Moreover, during the reproductive years, prominent smegma accumulation in males can cause vaginal infections in female partners, potentially resulting in recurrent infections for both individuals.Excessively long foreskin—particularly in cases of phimosis—favors smegma accumulation. Specifically, failure to clean the area regularly or prolonged inability to retract the foreskin leads to excessive secretion from the inner foreskin surface; over time, accumulated debris coalesces into clumpy, hardened smegma deposits.


In daily life, maintaining cleanliness of the genital area is essential. Proper cleaning requires full retraction of the foreskin to expose and cleanse the glans thoroughly. Mild saline solution may be used for cleansing, but strongly irritating chemical cleansers should be avoided, as they may provoke chemical balanitis. After washing, the foreskin should be promptly returned to its natural position to prevent paraphimosis. Men with redundant foreskin or phimosis should seek timely medical evaluation and consider circumcision.

Wear loose-fitting trousers and choose undergarments made of thin, breathable cotton fabric. If redness, swelling, or itching of the glans occurs due to smegma accumulation, avoid excessive anxiety—prompt medical consultation and treatment are recommended.
