Causes of recurrent epistaxis (nosebleeds) are categorized as local or systemic. Local causes include nose-picking, forceful nose-blowing, vigorous sneezing, and inappropriate intranasal medication use—all of which can damage the mucosal blood vessels in the nasal cavity. Inflammatory conditions affecting the nasal cavity and paranasal sinuses may impair capillaries in the nasal mucosa, leading to bleeding. Additionally, both benign and malignant tumors of the nasal cavity and paranasal sinuses can cause epistaxis.

What Causes Recurrent Nosebleeds?

Recurrent nosebleeds stem from either local or systemic factors. Inflammatory conditions affecting the nasal cavity or paranasal sinuses can injure capillaries in the nasal mucosa, leading to bleeding. Both benign and malignant tumors of the nasal cavity and paranasal sinuses may also cause epistaxis. Nasal septum disorders and atrophic rhinitis are additional local contributors. The most common local cause is nasal dryness—particularly during dry seasons—leading to desiccation of the nasal mucosa and subsequent epistaxis.

Given the numerous possible causes of recurrent nosebleeds, patients should not dismiss them lightly.Hematologic disorders—including thrombocytopenia and leukemia—may also trigger epistaxis.If nosebleeds occur frequently, prompt evaluation by an otolaryngologist is strongly recommended to identify the underlying cause. Systemic contributors include hypertension, coagulopathies, platelet abnormalities, and chronic diseases affecting the liver or kidneys.
