Why Does Epistaxis Occur with Internal Heat?
Epistaxis associated with internal heat may stem from either excessive liver fire or yin deficiency with hyperactive fire:
1. Excessive Liver Fire: This condition may result from chronic sleep deprivation due to prolonged late-night activity, or from excessive consumption of spicy foods. Affected individuals commonly experience recurrent internal heat and epistaxis, possibly accompanied by gingival swelling and pain, as well as dry mouth. Under medical guidance, medications such as Longdan Xiegan Wan (Gentiana Decoction for Draining Liver-Fire) or Chaihu Shugan San (Bupleurum Powder for Regulating Liver Qi) may be prescribed.
2. Yin Deficiency with Hyperactive Fire: This pattern—characterized by insufficient yin fluids leading to relative excess of fire—may arise from aging or, less commonly, from unhygienic sexual practices. Patients often exhibit irritability and recurrent epistaxis, along with symptoms such as insomnia, night sweats, restlessness, dry mouth, and sore throat. Under professional guidance, herbal formulas such as Xuan Mai Gan Ju Ke Li (Scrophularia-Ophiopogon-Glycyrrhiza-Platycodon Granules) or Liu Wei Di Huang Wan (Rehmannia Six Formula) may be used.
