Bilateral tinnitus has numerous causes, including disorders of the outer ear, middle ear, and inner ear. Generally, tinnitus caused by outer or middle ear problems is readily treatable. Bilateral tinnitus originating from the inner ear results from dysfunction of the auditory nerve, typically due to impaired microcirculation—such as ischemia, hypoxia, microthrombosis, or vasospasm—leading to diminished auditory nerve function.

What Causes Bilateral Tinnitus?

Bilateral tinnitus has numerous potential causes, involving the outer ear, middle ear, and inner ear. Generally, tinnitus resulting from outer or middle ear disorders is relatively easy to treat. In contrast, bilateral tinnitus originating in the inner ear is typically due to dysfunction of the auditory nerve, often caused by microcirculatory disturbances—including ischemia, hypoxia, microthrombosis, or vasospasm—leading to impaired auditory nerve function. Conditions that disrupt microcirculation include cerebral infarction, cerebral arteriosclerosis, cerebrovascular spasm or stenosis, hypertension, and diabetes. Additionally, certain structural abnormalities of the inner ear may also cause bilateral tinnitus—for example, compression by an acoustic neuroma or enlargement of the vestibular aqueduct.

Bilateral tinnitus may also be triggered by otitis media. If you have previously suffered from otitis media, residual infection or trauma may lead to secondary tinnitus. Furthermore, tinnitus can be either intermittent or persistent. After symptom onset, patients commonly experience significant hearing loss and congestion in the ears and nose; some severely affected individuals may even develop dizziness.

Patients with bilateral tinnitus should maintain emotional stability, ensure adequate and high-quality sleep, adhere to regular daily routines, avoid staying up late or becoming overly anxious or irritable, and refrain from excessive emotional excitement—since such factors may disturb sleep and thereby exacerbate tinnitus symptoms.
