Secondary pulmonary tuberculosis is not always contagious, but the risk of transmission is relatively high—primarily because it typically indicates an active phase of the disease. Whether secondary pulmonary tuberculosis is contagious depends on sputum test results; patients are therefore advised to undergo sputum culture testing—commonly performed as sputum smear microscopy in clinical practice. If Mycobacterium tuberculosis is detected in the sputum, the patient is considered infectious and highly contagious. There are two main subtypes of secondary pulmonary tuberculosis: one presents with gradual onset and mild clinical symptoms, carries a favorable prognosis, and poses a lower risk of transmission; the other progresses rapidly, causing extensive pulmonary lesions—including cavity formation and dissemination—in a short time, thereby increasing its transmissibility.

Secondary tuberculosis can spread via respiratory droplets—i.e., when an infected individual coughs or sneezes, M. tuberculosis may be expelled into the air during normal breathing. Healthy individuals who inhale these contaminated droplets may then become infected. Additionally, gastrointestinal and hematogenous (blood-borne) routes of infection are also possible.
