What Causes Ear Blockage?

Introduction
A sensation of ear fullness or blockage may result from cerumen (earwax) dislodging and obstructing the ear canal. Enlarged adenoids can compress and obstruct the pharyngeal orifice of the auditory (Eustachian) tube, leading to secretory otitis media and thereby causing a feeling of ear fullness. The inner ear is primarily responsible for hearing and balance functions; its blood supply originates from the labyrinthine artery. Impaired microcirculation in the inner ear or endolymphatic hydrops can also produce a sensation of ear fullness or blockage.

Ear fullness or blockage may result from cerumen impaction in the ear canal, but it can also be caused by conditions such as otitis media with effusion or sudden sensorineural hearing loss.

1. Cerumen Impaction

Cerumen (earwax) is produced by glands in the ear canal. When excess cerumen accumulates and fails to naturally migrate out of the ear canal, it forms a cerumen impaction, leading to a sensation of ear fullness or blockage.

2. Otitis Media with Effusion

Enlarged adenoids may compress and obstruct the pharyngeal orifice of the auditory (Eustachian) tube, resulting in otitis media with effusion. This condition causes accumulation of serous fluid—or even purulent exudate—in the middle ear cavity, producing a sensation of ear fullness or blockage.

3. Sudden Sensorineural Hearing Loss

The inner ear is primarily responsible for hearing and balance functions. Its blood supply originates from the labyrinthine artery. Compromised microcirculation in the inner ear—or pathological conditions such as endolymphatic hydrops—can lead to a sensation of ear fullness or blockage.

If you experience ear fullness or blockage, it is recommended that you consult an otolaryngologist (ENT specialist) for evaluation of the external auditory canal and tympanic membrane. Audiometric testing—including pure-tone audiometry and tympanometry—should also be performed to identify the underlying cause and guide appropriate treatment.
