In acute interstitial pneumonia, the disease is in the alveolar exudative phase, with no permanent damage to the pulmonary interstitium. However, if common irritants are removed or glucocorticoids are administered, the interstitial exudative pneumonia may progress chronically, resulting in irreversible damage to the alveolar architecture.

Interstitial pneumonia is a serious pulmonary disease whose severity depends on the underlying cause, extent of lung involvement, and timeliness and effectiveness of treatment. In acute interstitial pneumonia during the alveolar exudative phase, the pulmonary interstitium typically sustains no permanent damage. However, if causative irritants persist or if corticosteroid therapy is ineffective, the condition may progress chronically, resulting in irreversible structural damage to the alveoli. Fibroblastic proliferation may ensue, leading to cystic fibrosis of the alveolar septa, impaired pulmonary ventilation, and ultimately respiratory failure. Prognosis is generally poor, and the disease course is often severe. Another form—acute interstitial pneumonia—presents with rapid, diffuse interstitial lung changes over a very short period. It is unresponsive to pharmacotherapy, progresses swiftly, and can lead to respiratory failure and acute respiratory distress syndrome (ARDS) within days, posing an immediate threat to life.

Dietary management for interstitial pneumonia must emphasize light, easily digestible foods while strictly avoiding spicy, stimulating, greasy, and cold items. Smoking and alcohol consumption must be completely avoided. Patients should consume ample fruits and vegetables rich in vitamin C to bolster their immune defenses. High-quality protein sources—including milk, chicken, lamb, beef, fish, and shrimp—are also recommended. Because dyspnea is common in interstitial pneumonia, patients must avoid dietary irritants that could exacerbate coughing and impede recovery. Additionally, incorporating high-fiber foods into the diet promotes gastrointestinal motility and helps prevent constipation.
