Fasting is not required for rheumatoid factor (RF) testing. RF is an autoantibody commonly used in the diagnosis of rheumatoid arthritis (RA). RF is primarily produced in peripheral lymph nodes, synovial membranes of joints, tonsillar lymphoid follicles, and bone marrow. Food intake prior to testing does not affect RF measurement results. Elevated RF levels may indicate various conditions—both rheumatic (e.g., RA, Sjögren’s syndrome, systemic lupus erythematosus) and non-rheumatic (e.g., chronic infections, liver disease, malignancies). Notably, approximately 5% of healthy elderly individuals may test positive for RF, with positivity rates increasing with age. Careful differential diagnosis is essential to avoid misdiagnosis. In patients with confirmed RA, persistently high RF concentrations often correlate with greater disease activity. Therefore, fasting before RF testing is unnecessary.

For patients with rheumatoid arthritis, it is advisable to avoid spicy, stimulating, or certain fish-based foods—including beef, mutton, dog, donkey, and venison. Additionally, consumption of strong tea, strong coffee, and other stimulating beverages should be avoided. A low-salt, low-fat, and low-sugar diet is recommended, emphasizing light, easily digestible meals. A scientifically balanced diet with adequate nutrition is essential. Patients should include nutrient-dense, high-protein, calcium- and vitamin-rich foods such as milk, egg whites, lean meats, freshwater fish, black fungus, green leafy vegetables, and dark-colored fruits. Overweight patients should aim for weight loss by reducing intake of high-calorie and high-fat foods.
