Blood glucose control standards during pregnancy

Introduction
During pregnancy, for women without pre-existing diabetes, fasting blood glucose should not exceed 5.1 mmol/L, postprandial blood glucose one hour after meals should not exceed 10.0 mmol/L, and two hours after meals should not exceed 8.5 mmol/L. For women with gestational overt diabetes, fasting blood glucose should be controlled between 3.3–5.3 mmol/L, and two-hour postprandial glucose between 4.4–6.8 mmol/L.

Target blood glucose levels during pregnancy must be determined based on individual circumstances.

1. Women without preexisting diabetes

In pregnant women without diabetes, the recommended target glucose levels are as follows: fasting blood glucose should not exceed 5.1 mmol/L; 1-hour postprandial blood glucose should not exceed 10.0 mmol/L; and 2-hour postprandial blood glucose should not exceed 8.5 mmol/L.

2. Women with pregestational or overt gestational diabetes

For women with pregestational diabetes or overt gestational diabetes, fasting blood glucose should be maintained between 3.3–5.3 mmol/L, and 2-hour postprandial blood glucose should also be kept within 3.3–5.3 mmol/L.

If hyperglycemia develops during pregnancy, prompt and active management is essential. This typically involves controlling total daily caloric intake and incorporating appropriate physical activity to maintain blood glucose within the normal range. Prolonged hyperglycemia increases the risk of gestational diabetes mellitus and is associated with higher rates of miscarriage, preterm birth, and stillbirth.
