Syphilis rash, once it appears, is always systemic—even the palms and soles are involved. It never presents as a single lesion. However, numerous diseases can cause rashes; therefore, a rash alone cannot be used to diagnose syphilis. Definitive diagnosis of syphilis requires serological testing: both *Treponema pallidum*-specific antibodies and non–*Treponema pallidum*-specific antibodies must test positive on blood examination.

Skin syphilis rashes are not particularly specific, making it difficult to diagnose syphilis solely based on the rash appearance. So where on the human body can skin syphilis rashes appear?

Once they appear, syphilis rashes are systemic, meaning they can occur all over the body, including the palms of the hands and soles of the feet. They do not appear as a single isolated lesion. However, many diseases can cause rashes, so syphilis cannot be definitively diagnosed based on rash alone. To confirm syphilis, blood tests for both treponemal-specific antibodies and non-treponemal-specific antibodies are required. Only when both tests are positive can syphilis be confirmed; if the results are negative, syphilis is essentially ruled out.

Since syphilis rashes result from the dissemination of Treponema pallidum (the syphilis-causing bacterium) through the bloodstream to the skin surface, and because blood circulation reaches the entire body, these rashes can theoretically appear on any area of the skin. However, they have certain characteristics: due to the symmetry of blood flow on both sides of the body, the rashes typically appear symmetrically and tend to be widely distributed.
