Steps of tension-free inguinal hernia repair

Introduction

The specific surgical procedure involves first anesthetizing the surgical site, making an incision at the chosen location, wrapping and separating the hernial sac with saline-soaked gauze, removing the herniated contents, and then suturing the hernia defect after isolating the spermatic cord.

If a patient has an inguinal hernia, tension-free inguinal hernia repair can be selected for treatment. The specific surgical procedure is as follows:

1. Anesthesia: Inject procaine near the periosteum of the pubis and into the tissue surrounding the spermatic cord, performing rhomboid infiltration anesthesia between the two injection points;

2. Incision: Make an incision 2 cm above the inguinal ligament, starting slightly lateral to the midpoint of the inguinal ligament and extending obliquely toward the pubic tubercle, exposing the external oblique aponeurosis and the external ring;

3. Hernia sac dissection: Use hemostatic forceps to dissect the tissues, move aside the ilioinguinal nerve, then use straight scissors to cut the aponeurosis along its fiber direction, separating it into two flaps;

4. Resection of herniated contents: Pull the tissue to separate the cremaster muscle and expose the hernia sac. Then grasp the hernia sac with toothed forceps, select two adjacent sites, and make an incision between them;

5. Spermatic cord dissection: Use hemostatic forceps to grasp the edges of the incision, lift the hernia sac, and wrap the hernia sac and spermatic cord separately with gauze moistened with normal saline;

6. Hernia orifice closure: Free the neck of the hernia sac, incise it in the middle, reduce the contents of the sac back into the abdominal cavity, and then suture the opening at the neck of the hernia sac.
