An upset stomach upon waking in the morning while fasting may result from an extended fasting period, during which gastric acid is normally secreted and can irritate the gastric mucosa due to delayed eating. It could also indicate duodenal ulcer, typically caused by gastric acid and pepsin damaging the lining in a fasting state. Chronic gastritis may also cause these symptoms, often associated with long-term consumption of irritating beverages such as strong tea or coffee, or prolonged use of non-steroidal anti-inflammatory drugs (NSAIDs).

Feeling stomach discomfort on an empty stomach in the morning may be a normal physiological phenomenon, or it could indicate medical conditions such as duodenal ulcer or chronic gastritis.

1. Physiological Phenomenon

Morning stomach discomfort when fasting may result from prolonged fasting periods or early gastric emptying. Since gastric acid continues to be secreted normally, it may irritate the gastric mucosa. If food is not consumed in time, this may lead to physiological stomach pain. Symptoms typically improve significantly after eating.

2. Duodenal Ulcer

This discomfort might be caused by a duodenal ulcer. Such ulcers often occur under fasting conditions due to erosion by gastric acid and pepsin. They may also result from Helicobacter pylori infection or long-term use of steroid medications. The condition tends to recur and commonly presents with dull or burning pain in the stomach area.

3. Chronic Gastritis

The above symptoms may also arise from chronic gastritis. This condition is usually associated with long-term consumption of stimulants such as strong tea or coffee, or prolonged use of non-steroidal anti-inflammatory drugs (NSAIDs). It typically manifests as vague or burning stomach pain without a clear pattern.
