Manifestations of pre-hepatic coma

Introduction
Patients in the pre-coma stage of hepatic encephalopathy typically present with somnolence, and may also experience sleep reversal. Additionally, patients who are normally lively in personality may become depressed and introverted, whereas those who are usually introverted may exhibit a shift toward being more cheerful, outgoing, and talkative. Pre-coma stage patients may also show sudden flexion and rapid extension of the wrist joint, along with tremors and hyperkinesia.

Manifestations of the pre-coma stage of hepatic encephalopathy include changes in sleep patterns, personality alterations, and asterixis (flapping tremor).

1. Changes in sleep patterns

Patients with hepatic encephalopathy often experience excessive daytime sleepiness or sleep-wake inversion, such as sleeping during the day and becoming agitated at night. This is a relatively clear early sign of the pre-coma stage.

2. Personality changes

Patients who were previously lively may become depressed and withdrawn, while those who were originally introverted may become unusually cheerful, outgoing, and talkative.

3. Asterixis (flapping tremor)

In the pre-coma stage, patients may exhibit sudden flexion at the wrist joint followed by rapid extension, resulting in a tremor that resembles the flapping of a bird's wings. When the upper or lower limbs are affected, symptoms may include increased or decreased muscle tone and abnormal muscle contractions.

When patients show the above symptoms, it is recommended to seek timely medical evaluation to determine the underlying cause and receive appropriate treatment under medical guidance, thus preventing disease progression.
