Some pregnant women occasionally feel strong fetal movements and worry whether labor is about to begin, which may lead to anxiety and affect their daily lives. So, does intense fetal movement before delivery mean that labor is imminent?

If fetal movements are unusually strong before delivery, it is recommended to go to the hospital promptly for admission and observation. Obstetric evaluations should be performed, including color Doppler ultrasound during pregnancy and fetal heart rate monitoring, along with other related examinations. Pregnancy ultrasound can reveal whether the fetal heart rate, fetal movements, amniotic fluid, umbilical cord, and placenta are normal. Fetal heart rate monitoring helps assess the fetus’s functional reserve within the uterus.

If the above tests indicate fetal distress, it is recommended to administer oxygen while lying on the left side, along with medications to improve fetal heart function—such as high-concentration glucose combined with vitamin C and dexamethasone—to improve the intrauterine condition of the fetus. After active management, once the fetal heart rate shows clear improvement and fetal heart monitoring returns to normal, continued observation is appropriate. If conditions allow for vaginal delivery, a trial of vaginal birth may be considered. During labor, uterine contractions, fetal heart rate, fetal movements, and other conditions should be closely monitored, and timely decisions made regarding termination of pregnancy if necessary.

If fetal heart rate monitoring after the above interventions still indicates persistent fetal distress without significant improvement, and vaginal delivery within a short timeframe appears unfeasible, immediate cesarean section is recommended to terminate the pregnancy.
