The presence of epithelial cells in a urine test indicates those cells that have naturally sloughed off and died from the urinary tract lining. Elevated epithelial cells, especially when accompanied by high bacterial counts and increased white blood cells, may suggest a urinary tract infection. However, the cause of isolated elevated epithelial cells remains unclear. Even in healthy individuals without disease, epithelial cell levels can sometimes be elevated.

Nephritis is typically associated with hematuria (blood in urine), proteinuria (protein in urine), hypertension, and edema. The diagnosis primarily relies on findings of hematuria and proteinuria rather than elevated epithelial cells alone. Moreover, if proteinuria is present, it's important to determine whether it's acute or chronic nephritis, which requires additional tests for clarification. In cases of suspected chronic nephritis, a kidney biopsy may be necessary to identify the pathological type and assess prognosis. Isolated elevation of epithelial cells generally has little diagnostic value and cannot indicate nephritis; further evaluation alongside other test results is needed to determine the underlying cause.
