Video procedure of nephrostomy surgery
Percutaneous nephrostomy refers to creating a small opening from the skin into the kidney to artificially establish a passage through which urine from the kidney can be drained out of the body. The procedure must be performed at a正规 hospital, and the specific steps are as follows:
1. The patient lies supine on the operating table. After anesthesia is administered by an anesthesiologist, the surgeon performs routine disinfection and draping.
2. Once the anesthesia takes effect, the surgeon makes a puncture below the 12th rib margin at the intersection with the posterior axillary line, entering the posterior calyx of the kidney.
3. A metal guidewire is inserted through the puncture needle into the kidney and advanced into the ureter; then the puncture needle is withdrawn. A series of fascial dilators are threaded over the guidewire one after another to progressively dilate the tract.
4. A rigid plastic sheath dilator is advanced over the guidewire into the kidney. The surgeon holds the metal guidewire steady with one hand while withdrawing the dilator with the other, allowing a small amount of urine to drain through the plastic sheath.
5. A catheter is advanced over the guidewire through the plastic sheath into the kidney. After confirming the correct position of the guidewire within the kidney, the plastic sheath is removed first, followed by withdrawal of the guidewire.
6. The catheter is secured to the skin with sutures and connected to a urine collection bag.
Note: It is essential to confirm under X-ray guidance that the instruments have reached the correct position before removing the metal guidewire.
