Whether a fetus in abnormal position can be delivered vaginally depends on the fetal size and presentation. Vaginal delivery may be attempted if the fetus is small, weighing less than 2500 grams, and presents in left occiput anterior, right occiput anterior, or complete breech position, with manual version or postural correction to adjust fetal position. However, if the fetus is large, weighing more than 3000 grams, and in breech presentation, vaginal delivery is not possible. Attempting vaginal delivery under such conditions may lead to fetal asphyxia, maternal hemorrhage, or dystocia.

In general, whether vaginal delivery is possible in cases of abnormal fetal position depends on the baby's size and specific fetal presentation. The details are as follows:

1. Vaginal delivery may be possible

If the baby is relatively small, weighing less than 2500g, and the fetal position is left occiput anterior, right occiput anterior, or complete breech presentation, manual version (external cephalic version) performed by a skilled physician or maternal posture correction guided by a doctor may successfully reposition the fetus. In such cases, a trial of vaginal delivery is usually feasible.

2. Vaginal delivery is not recommended

If the baby is large, weighing more than 3000g, and presents in a breech position, vaginal delivery is generally not safe. Attempting vaginal delivery under these conditions may lead to complications such as fetal asphyxia, maternal hemorrhage, or obstructed labor. In such cases, cesarean section is recommended.
