What causes pain in the right upper back? Pain localized to the right side of the back or the shoulder–back region may stem from various underlying conditions. Back pain has numerous potential causes, among which muscle strain and lumbar or cervical disc herniation are the most common—both closely associated with daily lifestyle and occupational habits. Individuals engaged in prolonged desk work or heavy physical labor face a higher risk of developing these conditions.

Cervical disc herniation often arises secondary to cervical spondylosis, leading to compression of the cervical spinal cord, nerve roots, or vertebral arteries, thereby producing a constellation of functional impairments clinically recognized as a syndrome.

When cervical pathology is present, referred pain may extend to the right upper back, resulting in right-sided posterior back pain.
