Why hasn't my period come if the pregnancy test shows I'm not pregnant?

Introduction
Factors such as environmental changes, physical exertion, stress, staying up late, and taking medications may cause menstrual delay, resulting in absence of menstruation despite no pregnancy. Damage to the basal layer of the endometrium can lead to thinning of the endometrium, preventing its normal shedding. Polycystic ovary syndrome (PCOS) causes elevated levels of androgens in the body, disrupting normal ovulation and thereby affecting the menstrual cycle.

Pregnancy tests showing negative but menstruation not occurring may be caused by non-disease factors or by medical conditions such as thin endometrium or polycystic ovary syndrome (PCOS). It is important to identify the underlying cause promptly and seek appropriate treatment. Specific analysis is as follows:

1. Non-disease factors

Many factors can affect the menstrual cycle, including changes in environment, fatigue, stress, lack of sleep, or taking certain medications, all of which may lead to delayed menstruation despite a negative pregnancy test. A delay of up to one week is generally considered normal, and observation without intervention may be appropriate for now.

2. Disease-related causes

1) Thin endometrium

Procedures such as abortion or dilation and curettage (D&C) may damage the basal layer of the endometrium, resulting in a thinner uterine lining that fails to shed normally, leading to absent menstruation. Under a doctor's guidance, medications such as progesterone capsules or estradiol valerate tablets may be prescribed to regulate the condition.

2) Polycystic ovary syndrome (PCOS)

PCOS leads to elevated levels of androgens in the body, disrupting normal ovulation and thereby affecting the menstrual cycle, causing the aforementioned symptoms. Following a doctor’s advice, medications such as progesterone capsules or cyproterone acetate and ethinylestradiol tablets may be used to regulate menstruation.

In addition, ovarian function decline could also be a possible cause. It is recommended to visit a hospital promptly for examinations such as pelvic ultrasound and sex hormone panel (six-item testing), and receive targeted treatment based on the specific diagnosis.
