After a medical abortion, bleeding with blood clots for the first three days is caused by the expulsion of residual tissue and accumulated blood from the uterine cavity. If the bleeding exceeds normal menstrual flow and is accompanied by significant abdominal pain and large blood clots, it may indicate an incomplete abortion. Inadequate care can allow pathogenic bacteria to enter the uterine cavity, triggering inflammatory infection and resulting in the aforementioned symptoms.

Increased bleeding with blood clots three days after a medical abortion may be caused by normal physiological processes, or it could indicate pathological conditions such as incomplete abortion or uterine cavity infection. The specific analysis is as follows:

1. Physiological Phenomenon

After a medical abortion, bleeding typically lasts 3–10 days due to the expulsion of residual tissue and accumulated blood from the uterine cavity. Heavy bleeding with blood clots on the third day is usually caused by the discharge of intrauterine residues and is considered normal. The bleeding generally decreases gradually over time, so observation without immediate intervention is often acceptable.

2. Pathological Phenomenon

1) Incomplete Abortion

If the bleeding exceeds normal menstrual flow and is accompanied by significant abdominal pain and large blood clots, it may indicate an incomplete abortion. This condition often results in prolonged bleeding. Under a doctor’s guidance, medications such as Yimucao Granules, Xinshenghua Granules, or Shenghua Decoction can be taken to promote uterine contractions.

2) Uterine Cavity Infection

Following a medical abortion, the body's resistance is relatively low. Poor hygiene or inadequate care may allow pathogens to enter the uterine cavity, leading to inflammatory infection and causing the aforementioned symptoms, which are often accompanied by fever. Anti-inflammatory treatment with medications such as levofloxacin capsules, amoxicillin capsules, or metronidazole tablets should be administered according to medical advice.
