Introduction
Recurrent pain and splitting of the perineum may be caused by trauma, vulvar eczema, or vulvitis, and can be treated through proper intimate hygiene and medication. It could also result from vulvar white lesions, which can be treated with CO2 laser therapy. During treatment, maintain good perineal hygiene. If symptoms do not improve after treatment, timely consultation at a gynecology and obstetrics department is recommended.

Generally, recurrent splitting and pain in the perineal area may be caused by trauma, vulvitis, or vulvar eczema. Proper treatment includes maintaining good intimate hygiene and using medications. Specific details are as follows:

1. Trauma

If the perineal area is scratched or injured by a foreign object, it may cause local soft tissue damage and rupture of capillaries leading to bleeding. Without proper care of the genital area, the condition may recur. It is important to keep the vulva clean, wear loose and comfortable underwear, and avoid sexual intercourse during treatment to prevent interference with recovery.

2. Vulvar Eczema

This condition may result from an allergic reaction to certain hygiene products or frequent use of tight, non-breathable undergarments. After onset, localized allergic reactions can occur, causing redness, swelling, peeling, and cracking of the skin. Treatment may include topical applications such as calamine lotion, compound acetic acid dexamethasone cream, or mometasone furoate cream, used under medical guidance.

3. Vulvitis

Usually caused by bacterial infection, inflammation can lead to swelling and congestion of the vulvar mucosa. The affected skin may become dry, cracked, and painful. Some patients may also experience vulvar itching and increased vaginal discharge. Treatment typically involves antibiotics such as amoxicillin capsules, metronidazole tablets, or levofloxacin hydrochloride capsules, taken under a doctor's supervision.

In addition to the common causes mentioned above, recurrent symptoms might also be due to vulvar leukoplakia, which can be treated with CO2 laser therapy. Maintaining cleanliness of the perineal area during treatment is essential. If symptoms do not improve after treatment, prompt consultation with a gynecologist is recommended to avoid delayed treatment and worsening of the condition.
