Introduction
Hematemesis with a coffee-ground appearance may be caused by dietary factors, bile reflux, or gastric acid reflux. If large amounts of foods that are close to coffee in color—such as chocolate or black coffee—are consumed, these foods may be expelled along with gastrointestinal contents during vomiting, resulting in vomit that appears coffee-colored. Bile itself is yellowish-green; however, if the patient has underlying conditions such as biliary obstruction or cholecystitis, this may also contribute to the discoloration.

Vomiting blood that appears coffee-colored may be caused by dietary factors, bile reflux, or gastric acid reflux. The specific analysis is as follows:

1. Dietary factors

If a person consumes large amounts of foods with colors similar to coffee, such as chocolate or black coffee, these foods may be expelled along with gastrointestinal contents during vomiting, resulting in vomit that appears coffee-colored.

2. Bile reflux

Bile itself is yellow-green. If a patient has underlying conditions such as biliary obstruction or cholecystitis, bile metabolism may be impaired, leading to bile reflux. When bile mixes with blood, it may produce the aforementioned symptoms.

3. Gastric acid reflux

Infections, environmental factors, and other causes may irritate the gastrointestinal mucosa, resulting in excessive gastric acid secretion. When gastric acid refluxes due to abnormal function of the lower esophageal sphincter, this symptom may occur.

In addition, it could also be caused by gastric mucosal damage, liver cirrhosis, or other conditions. It is recommended that if discomfort symptoms occur in daily life, prompt medical attention should be sought to determine the exact cause and receive targeted treatment.
