The surgical procedure for adult umbilical hernia
Generally, adult umbilical hernia surgery can be classified into simple suture repair, open repair, and laparoscopic repair based on the surgical approach. Different surgical methods involve distinct procedures. Taking laparoscopic repair as an example, the specific steps are as follows:
1. The patient is placed in a supine position. The surgical site is disinfected with iodophor or medical alcohol, followed by anesthesia.
2. A 1 cm incision is made at the lower edge of the umbilicus to insert the laparoscope. The scope is advanced downward to create a preperitoneal space.
3. Incisions are made respectively at the lower-middle and upper one-third points along the line connecting the umbilicus and pubic symphysis for instrument placement.
4. The subcutaneous superficial fascia is incised to expose the anterior rectus sheath. The umbilical hernia sac is bluntly dissected free, and the sac wall tissue is removed.
5. Hemostatic forceps are used to lift and retract the area to examine for any vital organs or adhesions nearby.
6. After thoroughly cleaning the hernia sac, excess sac tissue is excised, the peritoneum is sutured, and the abdominal cavity is closed.
7. The fascial edges along the midline and the anterior rectus sheaths on both sides are overlapped. The lower flap is intermittently sutured in a mattress pattern beneath the upper flap, then the upper flap is laid over the outer surface of the lower flap and intermittently sutured to complete the repair.
8. The skin wound edges are trimmed and closed with interrupted sutures.
