Flatfoot is an extremely common condition. In its early stages, patients often experience pain on the anterior-medial aspect of the ankle joint. This pain typically worsens with prolonged standing or walking and improves with rest. Swelling and tenderness may also occur around the affected joint.

What Are the Consequences of Flatfoot?

1. Cosmetic and Aesthetic Impact

Individuals with flatfoot exhibit collapsed arches, resulting in abnormal gait patterns—both while walking and running. Specifically, they often adopt a “duck-footed” (out-toed) gait. For women, this gait can appear unrefined and significantly impair personal appearance; it may also hinder the ability to wear high-heeled shoes comfortably.

2. Impaired Ambulation

Due to arch collapse, individuals with flatfoot often cannot walk or stand for extended periods. Prolonged walking or excessive foot activity may lead to soft-tissue injury and subsequent inflammation. Uneven distribution of mechanical stress across joints further contributes to localized pain.

3. Associated Foot Disorders

Patients with flatfoot frequently experience excessive wear along the medial side of the heel. Prolonged standing or walking causes pain and impairs local circulation. Such biomechanical abnormalities predispose individuals to plantar fasciitis, Achilles tendinitis, patellar tendinitis, hip tendinitis, and hallux valgus.

4. Spinal Impact

The absence of adequate arch support in flatfoot results in elongated plantar ligaments. Under uneven mechanical loading, the heel tends to rotate inward during gait. Consequently, the calf muscles and knee joints experience asymmetric ligamentous strain, leading to inward bowing (genu valgum) and impaired lower-limb function. Compromised leg mechanics may subsequently affect pelvic alignment, potentially contributing to scoliosis as the pelvis shifts downward.
