1. Normal

Pigmented nevi form when melanocytes, during embryonic development, are interfered with by certain accidental factors while migrating from the neural crest to the skin, causing them to cluster locally within the skin. At birth, these pigment cells typically have not yet produced melanin and thus are not visible to the naked eye. As the person ages, due to stimulation from sunlight, ultraviolet radiation, and various hormones in the body, these melanocytes gradually become functionally activated and begin producing melanin, resulting in visible black moles. If the patient has no other discomfort symptoms, this is generally a normal phenomenon.

2. Abnormal

If a patient's pigmented nevus continues to increase in size and is accompanied by local symptoms such as itching or ulceration, this is considered abnormal and should be taken seriously. It could be filiform warts caused by human papillomavirus (HPV) infection. The typical presentation is single, thread-like, skin-colored or grayish-brown projections, which may also appear black, with a rough surface and firm texture. Patients may use topical medications such as fluorouracil cream or salicylic acid ointment as directed by a physician.
