The principle of the Heimlich maneuver involves applying external force to the patient's upper abdomen, causing the diaphragm to rise and thereby increasing intrathoracic pressure to create an outward thrust. If the patient is conscious and able to stand, the rescuer should stand behind the patient, wrap their arms around the patient’s upper abdomen, clasp their hands into a fist, and deliver upward abdominal thrusts. Repeat this action until the object is expelled. If the object cannot be dislodged after prolonged attempts, the maneuver has likely failed, and emergency services should be contacted immediately.

If the patient is unconscious, lay them flat on the ground and perform the same abdominal thrust technique. For pregnant patients, avoid aggressive abdominal compressions; instead, apply pressure to the lower half of the breastbone (sternum) in an attempt to dislodge the object. In such cases, emergency services should be called during rescue efforts to prevent serious consequences in case resuscitation fails.
