What does square shoulder deformity indicate in terms of injury, and how should it be managed?

Introduction
In general, square shoulder deformity suggests an injury, which may be related to trauma, shoulder dislocation, tuberculous arthritis of the shoulder joint, scoliosis, or local nerve lesions, and requires symptomatic treatment. If the shoulder is subjected to external force, the local soft tissues may be damaged, leading to symptoms such as redness, swelling, and pain. Without timely treatment, this can affect local muscle contraction and result in shoulder deformity.

Under normal circumstances, square-shoulder deformity suggests an injury and may be associated with trauma, shoulder joint dislocation, shoulder tuberculosis, scoliosis, or local nerve disorders, among other causes. Symptomatic treatment is required. Specific details are as follows:

1. Trauma: If the shoulder suffers external impact, local soft tissues may be damaged, leading to symptoms such as redness, swelling, and pain. Without timely treatment, muscle contraction in the area may be affected, resulting in shoulder deformity. Immediately after injury, patients can apply cold compresses with an ice pack to alleviate symptoms; after some time, heat therapy may help improve recovery.

2. Shoulder Joint Dislocation: Trauma to the shoulder may cause dislocation of the shoulder joint, resulting in severe pain, swelling, and deformity. Patients can benefit from manual reduction techniques, and should rest adequately while avoiding strenuous physical activity.

3. Shoulder Tuberculosis: An increased number of Mycobacterium tuberculosis in the body due to shoulder tuberculosis can disrupt the normal structure of the deltoid muscle, leading to square-shoulder deformity. Shoulder tuberculosis is mostly secondary, commonly following pulmonary tuberculosis or gastrointestinal tuberculosis. Patients are advised to follow medical instructions and take medications such as rifampicin capsules and isoniazid tablets for treatment.

4. Scoliosis: Scoliosis can have a genetic component—if parents have the condition, their children may have a higher risk of developing it. Common manifestations include asymmetric posture when standing and square-shoulder deformity. Patients may take celecoxib capsules or ibuprofen sustained-release capsules orally as directed by a physician to relieve pain, and orthotic bracing may also be used for correction.

5. Local Nerve Disorders: Nerve damage or numbness can cause pain in the soft tissues surrounding the shoulder joint. If nerve injury is not promptly corrected, pain around the shoulder muscles may progress to paralysis, eventually leading to muscle atrophy and resulting in square-shoulder deformity. Patients are advised to take medications such as mecobalamin tablets or vitamin B12 tablets under medical guidance to improve their condition. Additionally, a diet rich in high-protein and high-vitamin foods is recommended, while spicy foods and alcoholic beverages should be avoided.
