Taking medication on an empty stomach may lead to accelerated drug absorption, gastric mucosal irritation, abnormal gastrointestinal motility, altered hepatic metabolism, and allergic reactions.

1. Accelerated Absorption
Under fasting conditions, the absence of food in the stomach and small intestine eliminates interference with drug passage, allowing medications to be absorbed more rapidly across the gastrointestinal mucosa into the bloodstream—thereby exerting therapeutic effects sooner. For instance, certain antibiotics are absorbed more quickly when taken on an empty stomach, enabling them to reach effective plasma concentrations faster and exert their bactericidal and anti-infective effects more promptly.
2. Gastric Mucosal Irritation
Many medications possess inherent irritant properties toward the gastric mucosa. When taken on an empty stomach, these drugs come into direct contact with the unprotected gastric lining, lacking the buffering effect of food. This increases the risk of gastric mucosal injury, potentially causing symptoms such as epigastric pain, nausea, and vomiting.
3. Abnormal Gastrointestinal Motility
Some medications, when administered on an empty stomach, may disrupt normal gastrointestinal motility. This may result in excessively rapid peristalsis, leading to abdominal pain and diarrhea. Prolonged or inappropriate use may even contribute to chronic intestinal dysmotility.
