Luteal hematoma forms due to bleeding and blood accumulation following follicular rupture during ovulation. Whether it resolves spontaneously depends on the size of the hematoma. Generally, smaller luteal hematomas may resolve on their own and often do not require special treatment. In contrast, larger luteal hematomas typically do not resolve spontaneously and usually require medical intervention for symptom relief. The details are as follows:

1. Small luteal hematoma: Most luteal hematomas are physiological. When the hematoma is small, the condition is generally stable and can usually be absorbed and resolved spontaneously over time without specific treatment. However, regular follow-up at a hospital is necessary to monitor changes in the hematoma and prevent complications such as continued enlargement or spontaneous rupture with bleeding.

2. Large luteal hematoma: Larger hematomas may not be easily absorbed on their own. If not treated promptly, they can cause severe abdominal pain or even massive hemorrhage. Therefore, medications to promote absorption should be used under medical guidance, such as Leonurus granules, Xinshenghua granules, or Guizhi Fuling capsules. If drug therapy is ineffective, surgical removal of the hematoma may be considered. Additionally, if the corpus luteum cyst is large, there is a high risk of complications such as torsion or rupture, and timely surgical excision of the cyst is recommended.

During the period when a luteal hematoma is diagnosed, proper lifestyle management is also important.
