Self-Assessment Criteria for Geriatric Frailty

Manifestations of frailty include profound fatigue, unexplained weight loss, and recurrent respiratory or urinary tract infections. Falls also commonly occur; impaired balance and gait in older adults represent both a major risk factor for falls and a hallmark feature of frailty. In the frail state, even mild illness may compromise postural stability and impair gait integrity. Implementing timely, targeted preventive measures based on the severity of frailty—and strengthening self-care—can help delay its progression. Numerous studies have demonstrated a strong association between the degree of frailty and mortality in older adults: the more rapidly frailty progresses, the higher the mortality rate. Frailty is closely linked to other geriatric syndromes, including disability, dementia, sarcopenia, polypharmacy, reduced physical function, and sleep disorders. Moreover, frailty significantly increases the risk of adverse outcomes such as cardiovascular and cerebrovascular events, pulmonary infection, death, delirium, and falls. Knowledge Expansion: What Causes Geriatric Frailty?

1. Genetic Factors

Geriatric frailty is closely associated with genetic factors. Relevant genes include apolipoprotein E (ApoE), insulin receptor–like gene-2 (IRL-2), insulin receptor–like gene-16 (IRL-16), the coding region of C-reactive protein (CRP), and polymorphisms in the vitamin B12 gene (note: “vitamin B16” in original text appears to be an error; corrected here to vitamin B12, as no vitamin B16 exists).

2. Growth and Development

Accumulated physiological “reserves” established before full maturation significantly influence the development of frailty in later life. Adequate nutrition and regular physical activity—including occupational labor and structured exercise—during growth and development are critically important. Suboptimal growth and development lead to insufficient accumulation of these physiological “assets,” thereby increasing susceptibility to frailty in old age.

3. Multimorbidity

Research has shown that coronary heart disease, stroke, hip fracture, chronic obstructive pulmonary disease (COPD), diabetes mellitus, and arthritis are associated with increased frailty incidence three years later. Malignancies, renal failure, HIV infection, and surgical procedures can also accelerate the onset of frailty.

4. Malnutrition

Malnutrition is primarily characterized by inadequate daily energy intake. Studies indicate that older adults with poor nutritional scores—or those consuming fewer than three of the following nutrients (protein, vitamins A, C, and E, calcium, folate, and zinc)—exhibit significantly higher rates of frailty.
