Introduction
The key to successful resuscitation in acute cyanide poisoning lies in early diagnosis, immediate administration of antidotes, and supportive therapy—including specific detoxification treatments (Table 1) and general supportive measures such as oxygen supplementation, assisted ventilation when necessary, and maintenance of hemodynamic stability. On-site resuscitation should be continued; particularly when intravenous infusion is rapid, other antidotes—such as hydroxocobalamin—may also be employed.

Cyanides refer to compounds containing the cyanide group (–C≡N), including both inorganic and organic cyanides, which are widely used in industrial and agricultural production. Occupational exposure is the primary route of cyanide poisoning. Additionally, cyanogenic plants—such as apricot kernels, plum kernels, peach kernels, ginkgo nuts, and cassava—contain cyanogenic glycosides (precursors to cyanide). Once ingested, these precursors are metabolized in the body to produce hydrogen cyanide; excessive consumption may lead to cyanide poisoning. So, how is acute cyanide poisoning treated?

How Is Acute Cyanide Poisoning Treated?

The key to successful resuscitation from acute cyanide poisoning lies in early diagnosis and immediate administration of antidotes alongside supportive therapy—including specific detoxification measures (Table 1) and general supportive care such as oxygen supplementation, assisted ventilation when necessary, and maintenance of hemodynamic stability. On-site emergency management must be sustained. Combination antidote regimens carry multiple adverse effects, particularly when administered intravenously at rapid rates; common side effects include hypotension, tachycardia, and methemoglobinemia. Alternative antidotes include hydroxocobalamin, which chelates cyanide to form cyanocobalamin (vitamin B12), subsequently excreted by the kidneys. Cobalt-containing compounds (e.g., cobalt-EDTA) and other metal-based agents can also effectively promote cyanide elimination.

Knowledge Extension: Common Clinical Manifestations of Cyanide Poisoning

1. Central Nervous System

The CNS is predominantly affected. Inhalation of cyanide gas or ingestion of a large, lethal dose of potassium (or sodium) cyanide may cause sudden death within minutes. During cyanide poisoning, patients often emit a sharp cry before collapsing, rapidly losing consciousness and experiencing convulsions, followed by respiratory arrest and death. Some individuals report a sensation of throat constriction, headache, dizziness, nausea, and vomiting, progressing to loss of consciousness and agitation.

2. Respiratory System

Hypoxia ensues due to inhibition of cellular aerobic oxidation, resulting in enhanced anaerobic metabolism, increased lactic acid production, and elevated hydrogen ion concentration. Hydrogen ions readily cross the blood-brain barrier and directly stimulate central nervous system excitability, thereby increasing respiratory drive. Prolonged hypoxia reduces ATP production in respiratory muscles, impairing their contractility and leading to respiratory muscle fatigue. Cyanide can also damage alveolar epithelial cells, precipitating respiratory failure.

3. Digestive System

Nausea and vomiting occur because cyanide increases intracellular calcium influx, causing enhanced and spasmodic skeletal muscle contraction. Hypoxia elevates arterial partial pressure of carbon dioxide (PaCO2), stimulating the sympathetic nervous system and reducing digestive secretions while slowing gastrointestinal motility. This leads to gastric and intestinal stasis, indigestion, anorexia, and nausea.
