Currently, HSCT is widely applied in the treatment of malignant hematologic diseases, severe aplastic anemia, inherited metabolic disorders, and certain solid tumors, achieving favorable clinical outcomes. Hematopoietic stem cells possess the capacity for self-renewal and differentiation into all mature blood cell lineages, thereby sustaining lifelong hematopoiesis. These stem cells can be harvested from bone marrow, peripheral blood, or umbilical cord blood. With advances in modern medicine, HSCT has become an essential and highly effective clinical therapy. However, both patients with benign and malignant diseases may face disease relapse post-transplantation. 1. Infection
Following HSCT, patients experience significant immunosuppression, markedly increasing their susceptibility to infection—occurring in 50%–80% of cases. Causative agents include bacteria, fungi, viruses, and parasites. Infection remains one of the leading factors affecting long-term survival after transplantation.
2. Graft Failure
Graft failure is among the most serious early complications of HSCT. It occurs when the transplanted graft fails to engraft, resulting in failed hematopoietic reconstitution. Clinically, this manifests as severe pancytopenia accompanied by hypocellular or aplastic bone marrow, along with life-threatening infectious and hemorrhagic complications that are difficult to manage and associated with high mortality. However, with continuous improvements in transplant techniques, the incidence of graft failure has decreased to below 5%, and in some centers, even lower.
