Introduction
1. Antiallergic medications, such as epinephrine, exert effects including myocardial stimulation, blood pressure elevation, and bronchial smooth muscle relaxation. 2. Eliminate or avoid the allergen: - Cutaneous exposure (e.g., insect stings from bees, wasps, etc.); - Inhalational exposure (e.g., pollen allergy). 3. Administer timely fluid resuscitation; intravenous calcium gluconate may be given under medical supervision.

Anaphylactic shock is an extremely severe allergic reaction; without prompt resuscitation, death may occur within ten minutes in severe cases. Since the vast majority of anaphylactic shock cases are drug-induced, patients experiencing drug-related anaphylactic shock must immediately discontinue the offending medication and undergo continuous monitoring of blood pressure, pulse, and respiration. So, what are the management strategies for anaphylactic shock? The following addresses this question.

Management Strategies for Anaphylactic Shock

1. Antiallergic Medications

Epinephrine, for example, exerts positive inotropic effects on the myocardium, elevates blood pressure, and induces bronchial smooth muscle relaxation—thereby alleviating symptoms such as weak pulse, hypotension, and respiratory distress associated with anaphylactic shock. However, epinephrine must never be administered intravenously undiluted during anaphylactic shock; it must always be appropriately diluted. Undiluted intravenous administration carries a high risk of fatality. Additionally, establish intravenous (IV) access promptly. Hydrocortisone or methylprednisolone may be selected for slow IV infusion.

2. Removal of the Allergen  

Cutaneous exposure commonly occurs via insect stings (e.g., bee or wasp stings); inhalational exposure frequently involves pollen allergy; gastrointestinal exposure may trigger anaphylactic shock in sensitized individuals after ingestion of certain foods—such as eggs, shellfish, pineapple, peaches, or peanuts. If food ingestion precipitates anaphylactic shock, gastric tube placement is indicated, and gastric lavage may be necessary. Furthermore, intravenous exposure—particularly drug administration via injection—is the most common cause of anaphylactic shock.

3. Timely Fluid Resuscitation

Histamine release is the primary pathogenic driver of anaphylactic shock; therefore, antihistamine therapy is essential. Intramuscular injection of promethazine is commonly employed. For alert and conscious patients, oral cetirizine or desloratadine may be administered; alternatively, under physician guidance, 10% calcium gluconate (10–20 mL) may be given intravenously for antiallergic treatment. Note that 10% calcium gluconate injection must be diluted with an equal volume of 5%–25% dextrose injection prior to slow IV administration, at a rate not exceeding 5 mL per minute, to prevent rapid elevation of serum calcium levels and consequent cardiac arrhythmias.
