Can Strychnos nux-vomica be combined with Aconitum carmichaelii (black processed slice)?
If the patient's constitution is suitable and a doctor assesses that medication is necessary, Strychnos nux-vomica (Ma Qian Zi) can usually be combined with Aconitum carmichaelii (Hei Shun Pian). If the constitution is unsuitable or there are drug interactions, they generally should not be used together. If any discomfort occurs, prompt medical attention is recommended.

According to traditional Chinese medicine (TCM) theory, Strychnos nux-vomica has functions of dispersing nodules, relieving pain, and promoting circulation of Qi and blood in the channels, whereas Aconitum carmichaelii warms Yang to dispel cold and enhances fire to support Yang energy. When used together, these two herbs may theoretically exert synergistic effects—for example, in treating joint pain caused by cold-damp obstruction. If the patient's constitution is appropriate and under the guidance of a qualified TCM practitioner following the principle of syndrome differentiation and treatment, combining Strychnos nux-vomica with Aconitum carmichaelii may be safe and effective.
It should be noted that Strychnos nux-vomica contains highly toxic components, and improper use may lead to poisoning. If the patient has a weak constitution or presents with heat syndromes or Yin deficiency with hyperactive Yang, using Aconitum carmichaelii might worsen the condition. In such cases, even if the therapeutic effect of Strychnos nux-vomica is required, it should not be combined with Aconitum carmichaelii. Due to its high toxicity, Strychnos nux-vomica must be administered with strict dosage control. Even when combined with Aconitum carmichaelii, risks may arise due to dosage issues.
