Introduction
After the removal of a lower wisdom tooth, the opposing upper wisdom tooth loses its occlusal contact and may gradually over-erupt. An over-erupted upper wisdom tooth can bite into the lower gum or alveolar ridge, causing local pain and ulcers. It may also disrupt normal occlusion, leading to temporomandibular joint disorders, with symptoms such as joint clicking and difficulty opening the mouth. Maintaining good oral hygiene and undergoing regular dental check-ups are essential in daily care.

After removing the lower wisdom tooth, the upper wisdom tooth generally should not be left untreated, as neglecting it long-term may lead to various oral problems. Appropriate management should be carried out based on individual circumstances.

Without the opposing lower tooth to restrict its growth, the upper wisdom tooth tends to gradually over-erupt. An over-erupted upper wisdom tooth may bite into the lower gum or alveolar bone, causing localized pain and ulcers. It can also disrupt normal occlusion, leading to temporomandibular joint (TMJ) disorders, with symptoms such as joint clicking and difficulty opening the mouth.

Additionally, an over-erupted upper wisdom tooth often creates hard-to-clean areas between itself and adjacent teeth. Accumulation of food debris increases the risk of decay and periodontal disease in neighboring teeth. In severe cases, this may even result in root resorption of adjacent teeth, compromising their stability.

If the upper wisdom tooth is properly positioned and shows no sign of over-eruption, regular monitoring through check-ups may suffice. However, if over-eruption, tilting, or discomfort has already occurred, prompt medical attention and extraction are recommended to prevent further damage to oral health. Maintaining good oral hygiene and undergoing regular dental examinations are essential for early detection and timely management of potential oral issues.
