Management should be adjusted based on the time since injury and symptom progression to avoid worsening damage. If the bruising expands, pain intensifies, or the nail becomes loose, prompt medical evaluation at an orthopedic or emergency department is recommended.

  Nail injuries with bruising and pain due to finger trauma can be relieved through early cold compresses, later warm compresses, elevating the affected limb, topical medications, and moderate immobilization and protection. The approach should be adjusted based on the time since injury and symptoms to avoid worsening the damage. If the bruised area expands, pain intensifies, or the nail becomes loose, prompt medical attention at an orthopedic or emergency department is necessary.

  1. Early Cold Compress: Within the first 48 hours after injury, apply an ice pack or cold towel to the affected area for 15–20 minutes per session, 3–4 times daily. This constricts blood vessels, reduces bruising, and alleviates pain. Take care to avoid skin frostbite.

  2. Later Warm Compress: Begin using a warm towel on the injured area after 48 hours to promote local blood circulation and accelerate absorption of the bruise. Apply for about 15 minutes per session, ensuring the temperature is comfortable and does not cause burns, helping to reduce swelling.

  3. Elevate the Affected Limb: When resting, keep the injured finger elevated above heart level to reduce local blood accumulation and relieve swelling and pain. Avoid keeping the hand in a dependent (hanging down) position for prolonged periods, which may worsen discomfort.

  4. Topical Medications: Use blood-activating, stasis-resolving, anti-swelling, and pain-relieving topical agents as directed by a physician, such as safflower oil or Yunnan Baiyao spray. Follow instructions carefully to help alleviate pain and support recovery.

  5. Moderate Immobilization and Protection: Gently wrap the injured finger with sterile gauze or a bandage to prevent further impact or compression, minimizing external irritation and creating a stable environment for healing of the nail and bruising.

  Daily care should include keeping the injured finger clean and dry, avoiding contact with dirty water to prevent infection. Do not attempt to puncture or drain the blood under the nail yourself. Monitor recovery as instructed by your doctor and seek timely follow-up if abnormalities occur, so that treatment can be adjusted appropriately.
