Introduction
A persistent weak positive result on ovulation test strips in patients with endometriomas may be caused by improper testing timing, hormonal fluctuations, urine concentration, polycystic ovary syndrome (PCOS), or abnormal thyroid function. This can be improved by adjusting the testing time, following proper testing procedures, and medical treatment. If a strong positive result never appears or if symptoms such as abdominal pain or menstrual irregularities persist, prompt medical evaluation is necessary.

Ovulation test strips showing consistently weak positive results in patients with endometriomas (chocolate cysts) may be caused by improper testing timing, hormonal fluctuations, urine concentration variations, polycystic ovary syndrome (PCOS), or abnormal thyroid function. These issues can be improved by adjusting the testing schedule, following proper testing procedures, and medical treatment. If strong positive results never appear or are accompanied by abdominal pain or menstrual irregularities, prompt medical consultation is necessary.

1. Improper Testing Timing: Testing outside the ovulation window—either too early or too late—can result in hormone levels not reaching the threshold for a strong positive, leading to persistently weak test results. It is recommended to estimate the ovulation period based on the menstrual cycle and begin daily testing at the same time each day starting from day 10–14 of the cycle.

2. Hormonal Fluctuations: Endometriomas can impair ovarian function, resulting in insufficient or low peak luteinizing hormone (LH) secretion, which fails to trigger a strong positive response. Maintaining regular sleep patterns, avoiding late nights and excessive stress, can help reduce hormonal fluctuations.

3. Urine Concentration Effects: Drinking excessive fluids before testing may dilute urine, or improper use of first-morning urine can lead to inaccurate hormone concentration readings. It is advised to limit fluid intake for two hours prior to testing and preferably use first-morning urine to ensure testing accuracy.

4. Polycystic Ovary Syndrome (PCOS): Endocrine disorders in PCOS cause abnormal ovulation, with LH remaining at consistently low levels, resulting in weak positive test results, often accompanied by infrequent menstruation and hirsutism. Under medical guidance, patients may take medications such as dydrogesterone tablets, ethinylestradiol cyproterone acetate tablets, or clomiphene citrate capsules to alleviate symptoms.

5. Thyroid Function Abnormalities: Dysregulated thyroid hormone secretion can impair ovarian function and disrupt ovulation signals, causing persistently weak positive test results, along with symptoms like fatigue and weight changes. Under medical supervision, medications such as levothyroxine sodium tablets, methimazole tablets, or propylthiouracil tablets may be used to relieve symptoms.

Maintain a regular lifestyle, consume a balanced diet rich in protein and vitamins, engage in moderate exercise to improve physical health, avoid excessive anxiety, regularly monitor your menstrual cycle, and keep detailed records of test results for reference during medical consultations.
