After worsening, "small three positives" (chronic hepatitis B with specific serological markers) may progress to chronic hepatitis, liver fibrosis, cirrhosis, liver cancer, or liver failure. Persistent viral replication and long-term accumulation of liver damage are the main causes. If symptoms such as increased fatigue, jaundice, or ascites occur, prompt medical evaluation of liver function and viral load is recommended.

1. Chronic Hepatitis: The virus continuously replicates actively, repeatedly triggering inflammation by stimulating hepatocytes. This manifests as abnormal liver function tests. Long-standing inflammation gradually damages liver structure, and without timely intervention, the condition will continue to progress.

2. Liver Fibrosis: Following liver cell injury, the body initiates repair mechanisms, leading to excessive deposition of fibrous tissue that replaces normal liver cells. This results in decreased liver elasticity. At this stage, with appropriate control, the condition can be reversed; otherwise, it may advance to cirrhosis.

3. Cirrhosis: Progressive liver fibrosis leads to structural destruction of the liver and formation of pseudolobules, severely impairing liver function. Complications may include portal hypertension, ascites, and esophageal-gastric varices.

4. Liver Cancer: Chronic hepatitis B virus infection is a major risk factor for liver cancer. During prolonged liver inflammation and tissue repair processes, genetic mutations in hepatocytes may occur, eventually developing into malignant tumors. Early-stage symptoms are often subtle and easily overlooked.

5. Liver Failure: A rapid deterioration of liver function within a short period, resulting in the inability to maintain normal metabolism and detoxification. It presents as severe jaundice, coagulopathy, hepatic encephalopathy, and other life-threatening manifestations requiring urgent medical intervention.

Daily management should include regular作息 (sleep-wake cycles), avoiding staying up late, quitting smoking and alcohol to reduce liver burden, maintaining a light, balanced diet rich in high-quality protein and vitamins, undergoing regular follow-up tests including liver function and liver imaging, and strictly adhering to medical advice for standardized disease management.
