Introduction
Adolescents can develop myocardial ischemia—a condition not exclusive to middle-aged and elderly individuals. Although the incidence is relatively lower in adolescents, it may be triggered by unhealthy lifestyle habits acquired later in life or by specific underlying medical conditions. If an adolescent frequently experiences symptoms such as chest tightness, chest pain, or shortness of breath after physical activity—or any other related discomfort—prompt medical evaluation is essential to rule out cardiac issues.

The etiology of myocardial ischemia in adolescents differs significantly from that in older adults. Unlike in older populations, coronary atherosclerosis rarely plays a role. More common triggers include chronic sleep deprivation, excessive physical or mental fatigue, high-intensity strenuous exercise, severe anemia, and viral myocarditis. Prolonged disruption of circadian rhythms places sustained strain on the heart, substantially increasing myocardial oxygen demand and thereby disrupting the balance between myocardial oxygen supply and demand. Anemia impairs blood oxygen-carrying capacity, resulting in inadequate oxygen delivery to the myocardium; meanwhile, inflammatory damage compromises normal myocardial function, ultimately precipitating transient or persistent myocardial ischemia.

In most adolescent cases, myocardial ischemia represents a functional, reversible condition. With timely intervention and appropriate management, full recovery is typically achieved without permanent cardiac damage. However, persistent neglect of symptoms and continuation of unhealthy lifestyle patterns may lead to recurrent myocardial injury and progressively increased cardiac workload. Chronic myocardial hypoxia can impair normal cardiac development and functional maturation; in severe cases, it may trigger arrhythmias or elevate the risk of acute cardiac events—posing serious threats to adolescents’ overall health and physical development.

To help prevent myocardial ischemia, adolescents should adopt regular sleep–wake schedules (e.g., going to bed early and rising early), avoid chronic sleep deprivation and excessive physical exertion, and maintain a balanced diet to ensure adequate nutritional intake. Consistent, moderate, low-impact physical activity helps improve overall fitness and stabilize cardiovascular function—thereby effectively reducing the risk of myocardial ischemia.
