What Are the Treatment Options for Hepatic Ascites?
Ascites due to liver disease can be managed through various approaches, including daily rest and lifestyle adjustments, paracentesis (abdominal fluid drainage), diuretic therapy, human serum albumin support, and surgical shunt procedures. Ascites is a complication arising from severe hepatic injury and typically requires a multimodal, integrated treatment strategy. If symptoms such as rapidly worsening abdominal distension or altered mental status or behavior occur, prompt medical evaluation and treatment at a hospital are essential.
1. Daily Rest and Lifestyle Management: Following diagnosis, patients should ensure adequate bed rest to minimize physical exertion and facilitate hepatic tissue repair. Strict control of dietary sodium and fluid intake is critical to reduce water and sodium retention. A light, nutritionally balanced diet should be maintained, while daily weight and abdominal girth measurements should be recorded to monitor changes in ascites volume.
2. Paracentesis (Abdominal Fluid Drainage): When large-volume ascites causes significant intra-abdominal pressure and discomfort, therapeutic paracentesis may be performed to remove accumulated fluid. Careful regulation of the drainage rate and total volume removed is essential to prevent hemodynamic instability. Although this procedure provides rapid relief from symptoms such as chest tightness and abdominal distension, its effect is temporary.

3. Diuretic Therapy: Diuretics should be administered strictly per physician instructions to promote excretion of excess sodium and water, thereby reducing ascites formation. Serum electrolyte levels must be closely monitored, and diuretic dosages adjusted according to the degree of ascites resolution. Concurrent low-sodium diet adherence enhances therapeutic efficacy and helps prevent metabolic imbalances.
4. Human Serum Albumin Support Therapy: Impaired hepatic synthetic function leads to hypoalbuminemia, which exacerbates ascites accumulation. Intravenous albumin infusion increases plasma colloid osmotic pressure, promoting reabsorption of ascitic fluid into the vascular compartment. This approach synergizes with diuretic therapy to improve systemic fluid distribution.
5. Surgical Shunt Procedures: In cases of refractory ascites unresponsive to conservative measures, and when the patient’s overall condition permits, surgical shunt placement may be considered. Such procedures aim to reduce portal venous pressure—the primary driver of ascites formation. Comprehensive preoperative assessment is mandatory, and close postoperative monitoring of vital signs and laboratory parameters is required to promptly identify and manage potential life-threatening complications.
