Introduction

Hepatocellular carcinoma (HCC) can be managed through a comprehensive, multimodal approach—including daily lifestyle management and supportive care, local ablation therapy, transcatheter arterial chemoembolization (TACE), targeted and immunotherapy, and surgical resection. Treatment plans are individualized based on tumor stage, patient’s overall health status, and disease severity; different interventions are selected accordingly. Patients experiencing persistent severe right upper quadrant pain or rapid, unexplained weight loss should seek prompt medical evaluation and treatment.

1. Daily Lifestyle Management and Supportive Care: Patients with HCC should maintain regular sleep-wake cycles and ensure adequate rest to avoid excessive physical strain. A light, low-fat diet rich in high-quality nutrients is recommended. Alcohol, greasy foods, and mold-contaminated foods must be strictly avoided. Emotional stability and psychological well-being should be prioritized to minimize stress-induced fluctuations, thereby enhancing the body’s tolerance to treatment and supporting clinical interventions.

2. Local Ablation Therapy: This minimally invasive physical modality is suitable for small, early-stage tumors. It employs thermal (e.g., radiofrequency or microwave) or cryoablation techniques to directly destroy tumor tissue, inducing coagulative necrosis of cancer cells. The procedure involves minimal incisions and causes limited damage to surrounding healthy liver parenchyma, enabling rapid postoperative recovery—making it especially appropriate for early-stage patients with compromised physical reserves.

3. Transcatheter Arterial Chemoembolization (TACE): A minimally invasive locoregional therapy, TACE selectively occludes tumor-feeding arteries to deprive the tumor of its blood supply and essential nutrients, thereby inhibiting tumor growth and proliferation. It is commonly used in intermediate- to advanced-stage HCC patients who are not candidates for surgery. TACE helps delay disease progression, control tumor burden, and prolong overall survival.

4. Targeted and Immunotherapy: For intermediate- to advanced-stage HCC, systemic pharmacotherapy using targeted agents (e.g., tyrosine kinase inhibitors) and/or immune checkpoint inhibitors is employed. These drugs precisely inhibit tumor cell proliferation and simultaneously activate the host’s intrinsic immune response against malignancy. Pharmacologic treatment stabilizes disease, reduces tumor volume, and alleviates associated symptoms such as fatigue, anorexia, and abdominal discomfort.

5. Surgical Resection: Surgery remains the primary curative option for early-stage HCC. It involves complete excision of the tumor along with a narrow margin of adjacent at-risk hepatic tissue. Ideal candidates have localized disease, preserved liver function reserve (e.g., Child-Pugh class A), and no evidence of extrahepatic metastasis. Surgical resection offers the best chance for long-term disease-free survival and clinical cure.

Maintain a calm and optimistic mindset in daily life; adopt a pattern of eating smaller, more frequent meals to reduce hepatic metabolic load; engage in gentle, moderate physical activity; undergo regular follow-up examinations to monitor tumor dynamics; and avoid lifestyle habits that exacerbate liver injury (e.g., alcohol consumption, unregulated herbal remedies, or exposure to hepatotoxins).
